IVF for Unmarried Couples in the UAE: Required Documents and Approval Process
Important: Fertility treatment for unmarried couples is available in the UAE only for non-Muslim partners and requires prior approval from Dubai Health Authority (DHA).
At Samaa Fertility, we guide our patients through every step of the documentation process and assist with preparing the application package for DHA approval.
What documents are required?
1. Parentage Declaration Form
This is the primary document required for DHA approval.
The form will be provided by our clinic and must be signed by both partners in person. Once completed, it must be legalized with:
A stamp from your country's Embassy or Consulate in the UAE;
A stamp from the UAE Ministry of Foreign Affairs (MOFA).
The MOFA attestation request can be submitted online, after which a courier service collects and returns the legalized documents.
2. Official Letter from the Medical Facility
This document is prepared and provided by Samaa Fertility.
3. Consent Form for Cryopreservation
This form will be provided and signed at our clinic.
4. Informed Consent for Fertility Treatment
This form will be provided and signed at our clinic before treatment begins.
5. Current Medical Reports
Relevant medical reports and treatment summaries for both partners will be prepared by our clinic for submission to DHA.
6. Identification Documents
Please provide:
Copies of Emirates IDs (if applicable);
Copies of passports for both partners.
7. Proof of Religion
A signed letter confirming that both partners are non-Muslims. The letter may be prepared in a free format and must be signed by both individuals.
How long does the approval process take?
Once all required documents have been collected, the application can be submitted to DHA for review.
DHA approval process typically takes approximately one week.
Once issued, the DHA approval is valid for three months.
